Enhanced Care Management (ECM) is a Medi-Cal managed care plan benefit that provides coordinated, whole-person support for children and families with complex needs. ECM is designed to meet families where they are.
Many families navigate multiple systems—medical, behavioral health, school, and community resources—and ECM helps bridge those gaps by building trusted relationships and aligning care with each child’s needs, strengths, and goals.
We are contracted with Kaiser Permanente, Health Net, Blue Shield Promise, Anthem Blue Cross, LA Care, and Molina to deliver ECM services.
Overall, ECM helps make care feel simpler, more connected, and easier for families to navigate.

Who We Serve
Enhanced Care Management supports children and families who need extra coordination due to complex health and life circumstances, including:
- Children with multiple medical and/or behavioral health needs who receive care from several providers or specialists
- Families supporting children with ongoing emotional, developmental, or behavioral health needs that impact daily functioning and care consistency
- Children with recent or frequent emergency room visits, hospitalizations, or difficulty staying connected to ongoing care
- Families experiencing care that feels fragmented or difficult to coordinate across medical, behavioral health, school, and community systems
- Situations where families need support connecting services and following through with recommended care across providers and settings
- Families facing barriers to accessing or maintaining care, such as transportation challenges or difficulty attending appointments

- Dedicated Lead Care Manager You Can Trust
Your family is paired with a consistent Lead Care Manager who builds a relationship with you and supports your child’s medical, behavioral, and social needs throughout their care journey.
- Coordinated Care Across Providers and Systems
We help connect and organize care between your child’s doctors, specialists, and support services so everything works together in a clear and coordinated way.
- Support During Key Transitions and Changes
We help your family navigate important moments such as hospital discharges, new diagnoses, or changes in care needs, helping ensure continuity and stability in care.
- Help Understanding and Navigating Care
We support families with understanding care recommendations, scheduling appointments, and communicating with providers so they feel informed, supported, and confident in their child’s care.
- Support Addressing Barriers to Care
We work with families to identify and reduce challenges that may impact access to care or follow-through with recommended care plans, helping improve consistency and outcomes.

Getting Started
Enhanced Care Management is available for children enrolled in Medi-Cal managed care plans, including Kaiser Permanente, Health Net, Blue Shield Promise, Anthem Blue Cross, LA Care, and Molina.

ECM Family Support Request
Enhanced Care Management (ECM) services are available for eligible children, youth, and adults residing in Los Angeles County who have Medi-Cal managed care plan and need extra support with health, emotional well-being, or complex life challenges.
Please note: This form is intended for one child, youth, or adult per submission. If you are requesting ECM services for more than one person in your household, please complete a separate form for each individual. This helps us provide timely and accurate support. Caregivers of children and youth who qualify for ECM are also eligible to receive support.
